This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.

1. Our Commitment to Your Privacy

Fix Your Feet Inc. ("we," "us," or "the practice") is required by law to maintain the privacy of your protected health information ("PHI"), to provide you with this Notice of our legal duties and privacy practices with respect to your PHI, to follow the terms of the Notice currently in effect, and to notify you following a breach of unsecured PHI. PHI is information about you, including demographic information, that may identify you and that relates to your past, present, or future physical or mental health and related health care services.

2. How We May Use and Disclose Your Health Information

The following describes the ways we may use and disclose your PHI without your written authorization:

Treatment

We use and disclose your PHI to provide, coordinate, and manage your podiatric care and related services. For example, Dr. Ragland and clinical staff may review your intake information, foot photographs, X-rays, and medical history to plan and perform a procedure, and may share information with another physician, surgery center, pharmacy, or specialist involved in your care.

Payment

We use and disclose your PHI to obtain payment for the services we provide. For example, we may share information with your health insurer or plan to verify coverage, obtain prior authorization, and submit claims for the care you receive.

Health Care Operations

We use and disclose your PHI to run the practice, improve the quality of care, train staff, and conduct business management. For example, we may use your information to review the performance of our clinical team or to contact you with appointment reminders.

3. Uses and Disclosures That Require Your Written Authorization

The following uses and disclosures will be made only with your written authorization:

  • Most uses and disclosures of psychotherapy notes (where applicable);
  • Uses and disclosures of PHI for marketing purposes;
  • Disclosures that constitute a sale of PHI; and
  • Other uses and disclosures not described in this Notice.

Your right to revoke. If you give us a written authorization, you may revoke it in writing at any time by submitting a written request to our Privacy Officer (contact details below). The revocation will stop any future uses or disclosures made under that authorization, except to the extent we have already acted in reliance on it.

4. Disclosures Permitted Without Authorization

We may use or disclose your PHI without your authorization in the following circumstances, subject to applicable legal limits:

  • When required by federal, state, or local law;
  • For public health activities (e.g., reporting disease, injury, or vital events; reporting to the FDA);
  • To report suspected abuse, neglect, or domestic violence;
  • For health oversight activities such as audits and inspections;
  • In response to a court or administrative order, subpoena, or other lawful process;
  • For specialized law enforcement purposes;
  • To coroners, medical examiners, and funeral directors;
  • For organ and tissue donation;
  • For research, where permitted with appropriate approvals and safeguards;
  • To avert a serious and imminent threat to health or safety;
  • For specialized government functions (e.g., military, national security); and
  • For workers' compensation as authorized by law.

5. Your Rights Regarding Your PHI

You have the following rights with respect to your PHI:

  • Right to access and copy. You may inspect and obtain a copy of your PHI, in the form and format you request when readily producible. We may charge a reasonable, cost-based fee.
  • Right to amend. You may request that we amend PHI you believe is incorrect or incomplete.
  • Right to an accounting of disclosures. You may request a list of certain disclosures we have made of your PHI.
  • Right to request restrictions. You may request a restriction on certain uses and disclosures; we are not required to agree except where the disclosure is to a health plan for a service you paid for in full out of pocket.
  • Right to confidential communications. You may request that we contact you by alternative means or at an alternative location.
  • Right to a paper copy of this Notice. You may obtain a paper copy upon request, even if you agreed to receive it electronically.
  • Right to breach notification. You will be notified following a breach of your unsecured PHI as required by law.

6. Our Duties

We are required by law to maintain the privacy and security of your PHI, to abide by the terms of the Notice currently in effect, and to notify you in the event of a breach of your unsecured PHI. We encrypt PHI at rest and limit access to staff who need it to deliver and administer your care.

7. New York Confidentiality Addendum

Because we provide services in New York, certain categories of your health information receive heightened protection under New York law in addition to HIPAA:

  • HIV/AIDS-related information is protected under New York Public Health Law Article 27-F. We will not disclose such information except with your specific authorization or as otherwise permitted by that law, and recipients are limited in their ability to redisclose it.
  • Mental health information is protected under New York Mental Hygiene Law §33.13 and is subject to additional limitations on use and disclosure.

8. Complaints

If you believe your privacy rights have been violated, you may file a complaint with our Privacy Officer using the contact details below, or with the Secretary of the U.S. Department of Health and Human Services, Office for Civil Rights. You will not be retaliated against for filing a complaint.

10. Changes to This Notice

We reserve the right to change this Notice and to make the revised Notice effective for PHI we already have as well as any information we receive in the future. The current Notice will always be posted on this page with its effective date. This Notice is effective as of June 7, 2026.
